Acme Health Services, Inc. partners with various Visiting Physician groups who can provide home visits by a medical doctor or nurse practitioner when necessary.
It is our goal to improve home care delivery in the comfort of the patient’s home, whether that be a private residence, skilled nursing facility, assisted living community or adult family care home. By delivering medical care and quality home health services to our patients, we are able to assist them in saving both time and money by avoiding unnecessary ambulance expenses and emergency department visits which may lead to hospitalization. Our home care services are also designed to allow for patients to age in place, and therefore, allow them to avoid premature Nursing Home placements.
